hi, thanks for adding me. Yes I know two companies that do apps, depends on your requirements. Look at cobiinteractive (http://www.cobiinteractive.com) and devicemagic (devicemagic.com). Cobi will build from scratch, device magic has a very clear way of creating data capture forms for mobile... again depends on what your requirements are ..
